I authorize GMA to charge my credit card monthly dues of (19.95 ) non-refundable. GMA requires a Three month comittment. I understand that failure to pay monthly dues and/or purchased product or marketing material will cause my account to be suspended and/or terminated by GMA and my account will be sent to collections.

I understand that it is my responsibility to collect and submit any taxes required of my county, state and federal agencies. I understand that I must advice Gift Marketing Alliance of my local tax rate and keep it updated at all times.

I understand that if I am going to market alcoholic gift baskets on my website that I must have the legal alcohol statement on my website.

I understand that I may not use the word Godiva or any form of Godiva when doing marketing or pay per click marketing.

I understand that before any voluntary or involuntary separation from Gift Marketing Alliance, I must remove all images from my website.

I understand that I may discontinue my membership at any time, by written request only.
